が、酸素吸入の限界を認識して、本当は何が有益なのかを教示していると私は感じる。これはモルヒネ吸入に置き換えても基本は同じであると考える。

結論

症状マネージメントは、その人らしく生き抜くQOLのために治療の限界を知りつつ、患者にとって何が有益かをいつもお互いに捜さなければならない。痛みと同様Total Dyspneaと呼ばれ、癌終末期の半数以上に見られ複雑な成因の呼吸困難にモルヒネ吸入を試みた。

モルヒネ吸入は肺内のオピオイドレセプターを介して末梢から作用すると想定された。

1．有効性

1）効果が速い

2）全身投与よりも少量ですむ

3）全身投与に追加して頓用で調節できる

4）水分関与のリンパ管症や胸水のときに有益だった

2．問題点

1）有効気道面積が少ない時は有益でなかった

2）不安パニックには有益でなかった。

3）気道刺激性が見られた
